    Form to Make Changes to Medical Plan: CalPERS Medical Enrollment Form HBD 12  (FOR OPEN ENROLLMENT ONLY) 

    General Information

    • COST: For full time employees represented by ATA, CSEA, and SEIU bargaining units, the district will provide payment of medical premiums up to the Kaiser family plan plus $100, the Delta Dental family plan, and the VSP vision plan. For part-time employees, the district will provide payment of medical premiums at the same percentage as his/her full-time equivalency (FTE).

    • MEDICAL: The employee may choose any available CalPERS medical plan, however the employee shall pay for any cost of the plan that exceeds the Kaiser HMO family plan plus $100.

    • DENTAL: The plan name is Delta Dental Premier, group number 7046-0009. The plan is part of an incentive program, meaning there is incentive to use the benefits. During the first year, Delta pays 70% of all covered procedures/benefits (including preventative care), the member pays 30%. For every calendar year in which the plan benefits are used, Delta increases their copay by 10% and decreases the member's copay by 10% until the member reaches 100% coverage of all contracted benefits. There is a calendar maximum of $2,000.00 per person. Please note: Each covered dependent is on their own incentive track and advances incentive levels only if the benefits are used within the calendar year. Delta Dental Link 

    • VISION: The vision coverage is offered through VSP. The benefits include one refractive eye exam per 12 months, at a participating VSP doctor, with a $5 copay. Included in the $5 copay are single vision, lined bifocal, and lined trifocal lenses, as well as photochromic and tinted lenses. There is an allowance of $120 towards frames and a discount towards any amount over the allowance. Alternatively, there is a $105 allowance towards a contact lense fitting and contact lense prescription. Detailed benefit information can also be found on the VSP website below.     ADDITIONAL RETIREMENT OPTION:

    • 403(b) & 457(b) RETIREMENT: AUSD employees with FTE assignments are eligible to enroll in 403(b) and 457(b) retirement savings plans, which allows you to make pre-tax contributions in the form of payroll deductions, and that money grows tax-deferred until retirement.

    • 403(b) ROTH:  Your contributions are made on an after-tax basis; taxes on the contributed amount are paid up front and there is no current tax break.  Contributions and earnings grow tax-free and can be withdrawn tax-free at retirement if certain conditions are met.

    • Enrollment procedures/steps:

      • Choose an investment provider listed on the link above.

      • Contact the investment provider directly to establish a 403(b) and/or 457(b) account or 403b ROTH account.

      • Submit relevant Salary Reduction Agreement(s) to TSACG (online form can be found in the TSA link above and PDF version below).  They will contact the Payroll Department to begin deductions.  (You must contact them to begin/end or change the amount of your contribution through your payroll deductions.)

    • Apple Retirement: For non FTE employees, you are automatically enrolled in Apple Retirement if you are not a CalSTRS or CalPERS member.  The district provides a matching 3.75% matching contribution to your contribution/deduction from payroll.

     

    • TAX WITHHOLDING: Submit a W4 for Federal Tax Withholding changes and DE4 for California Tax Withholding changes.   Effective January 1, 2020, you must submit DE4 also if you are making changes to your state taxes as well.  

    • DIRECT DEPOSIT: Direct deposit is the preferred method of payroll payments.  It's more secure, efficient, and on-time compared to hard copy paychecks through the mail.  To enroll in direct deposit into your checking/savings account, submit the Direct Deposit Form found below along with a copy of your voided check or online direct deposit instructions from your bank to payroll.
