The School Exit Form is required to dis-enroll a student from Albany Unified School District. AUSD is legally required to report to the state for each student exiting. Please fill out one form for each student leaving Albany Unified School District.

Examples:

• Moving out of state, out of the country, or moving to another city in California; or
• Enrolling in a private school.

The School Exit Form is NOT needed to request a change from one Albany elementary school to another (School Change Request is here), or when a student is progressing from elementary to middle school, or from middle school to high school.
